Rehab Group of Morristown
Dear Parents/Coaches,
This letter is intended to inform you of MHHS East/West pre-participation physical examinations. These physicals will be administered on from 9 am to 1 pm, Friday April 19, 201 9 . The physical examination will be conducted at Rehab Group of Morristown (www.rehabgrouptherapy.com)
The cost will be $20 per athlete.
We will again be using Dragonfly Max to ensure the health and safety of your child and their health information. Dragonfly Max (www.dragonflymax.com) is an athlete-centered electronic medical record focused on improving health, performance, and communication. This platform simplifies the task of completing preseason paperwork by allowing you to complete these health screening forms electronically.
It is imperative that you complete the registration process, through Dragonfly Max, before coming to the Rehab Group Physical scheduled on April 20th. Please complete the required forms online if you are a new athlete. If you are a returning athlete, simply update your account with current information.
Follow the simple steps on the next page to begin completing your child's forms. It is important that your athlete's medical heath screening and emergency contact information forms prior to the exam.
*NO athlete will be allowed to participate in any 2019-2020 school sport without a physical dated after April 15, 2019.
* NO female athlete will receive a physical without the presence of an adult female parent or coach.
